Blackhawk Museum Group Tour Request Form

Please complete this request form for your Adult/Senior Group Tour and submit it four (4) weeks prior to your preferred tour date. All Guided Tours require a 10 person minimum and are one hour in duration. Two tours, per group, per day are available. Guided Tours are subject to Docent availability. Self-Guided tours with 10 people or more are required to submit a Group Tour Request Form. Group admission pricing is extended if tickets are paid in advance or with one group ticket purchase at the Reception Desk at the time of the tour. The Programs Department will be in contact with the person making the tour request.
