Recent news concerning Blackhawk Plaza does not affect the Blackhawk Museum. We want to assure our community that the Blackhawk Museum operates under separate, independent ownership and is not closing.

Disabled Group Tour Request Form

Please complete this request form for your Disabled Group Tour and submit it four (4) weeks prior to your preferred tour date. All Guided Tours require a 10-person minimum and are one hour in duration. Guided Tours are subject to Docent availability. Self-Guided tours of any number of people are required to submit a Disabled Group Tour Request Form for Friday, Saturday, or Sunday only.

There is no charge for Disabled Group Tours. A member of our Programs Department will be in contact with the person making the tour request.
